Means of Requesting Disclosure
Requests for disclosure may be made by mail, telephone, or fax to the Company. Requests must be made by submitting a written request form and a document verifying the identity of the applicant. Click hereto download the request form. (The request form can also be ordered by phone or fax.
Identification documents
A driver’s license, health insurance card, or passport (photocopies are acceptable). Documents submitted will not be returned. Personal information on the submitted documents will be destroyed by incineration, shredding, or other means as soon as the contents are confirmed and verified, and a response is received.
Confirmation of proxy
A letter of attorney indicating representation will be considered as confirmation.
Time to Disclosure
We will send a written response within one week of receipt of the disclosure request documents.
Method of disclosure
Written documents will be delivered by the department in charge. However, with the consent of the requesting customer, we will also provide a verbal explanation, telephone explanation, or reply by e-mail.
Refusal of disclosure
In any of the following cases, we may refuse to disclose all or part of the information. In such cases, the customer will be notified to that effect.
(1) When there is a risk of harm to the life, body, property, or other rights or interests of the customer or a third party.
(2) If there is a risk of significant hindrance to the proper execution of the business of the business operator handling the personal information
(3) If it would violate other laws or regulations

Correction of Contents
When we receive a request from a customer that the contents of his/her personal information are not true, we will fully investigate the facts without delay, correct the customer’s information based on the results, and notify the customer that the correction has been made.
Suspension of Use
If a customer requests that we stop using his/her information for purposes other than those for which he/she has not consented, or for improper acquisition or provision to a third party without his/her consent, we will promptly take the relevant measures. When the suspension of use is also implemented, we will notify the customer to that effect.
